Are you a passionate and experienced Social Worker ready to make a real impact in Adult Social Care? We're looking for someone just like you to join a dynamic Hospital Discharge to Assess Team in Greater Manchester. ✅ What You’ll Be Doing:

• Leading on assessments, care planning and discharge coordination
• Making sound decisions using your professional knowledge and experience
• Working closely with NHS colleagues, therapists, providers and families
• Delivering safe, person-centred care in line with legislation and best practice
• Managing a caseload while ensuring timely, effective interventions
• Supporting less experienced staff and students

 
👤 What We’re Looking For:

• Social Work England registered
• Degree (or equivalent) in Social Work
• 3+ years' experience in Adult Social Care (Hospital/Discharge experience ideal)
• Skilled in assessments, care planning and risk management
• Confident working in a fast-paced, multi-agency environment
• Passionate about delivering the best outcomes for vulnerable adults
• Strong communication, mediation and relationship-building skills
