Pay rate £32.55 per hour
Contract role
Vitalis are recruiting for an experienced Social Worker to work in a hospital team in Westminster Council.
 
Key Responsibilities:

• Triaging referrals on behalf of the Tri-Borough Hospital Discharge Team
• Completing quick assessments for residents of the Tri-Borough
• Undertaking Care Act Assessments and developing Support Plans
• Completing Safeguarding Concern Assessments (Section 42.1)
• Setting up care packages to facilitate safe discharge
• Providing information and signposting to A&E staff

 
Essential Requirements:

• Social Work England Registration
• Proven experience in hospital discharge work
• Strong knowledge of the Care Act and safeguarding procedures
• Ability to work in a fast-paced environment and manage competing priorities
