Job Description

Health and Social Care Officers (HSCOs) Nottingham City 
Responsible for triaging, co-ordinating and providing information and advice to citizens and professionals over the phone and in response to our on-line referral process. This is to enable decision making and risk management on behalf of citizens requesting access to Adult Social Care. HSCOs support citizens and professionals to navigate through to the right services based on a telephone assessment of their needs. HSCOs assist in the delivery of an efficient and effective referral service for vulnerable adults, sometimes with complex needs and /or difficult and unpredictable situations. Applicants should have a good understanding of the variety of services that are available to meet the individual's social care needs. The role is classed as hybrid with a 50/50 mix of office and home working but will involve an initial period of full time office based work in our Bilborough offices until we are confident in the HSCO's abilities and understanding of the role. 
£19.10 per hour
