Job Description

Adult Social Worker – Hospital Discharge Team – Darlington
Pay rate £27 per hour
Your role as a Social Worker:
To complete facilitate hospital discharges and review following period of recovery.
To complete reablement assessments, CSNA, MCA, risk assessments, Safeguarding, CHC and DST's.
