To provide high-quality, person-centred occupational therapy within the Contact Team (Front Door Services), focusing on timely screening of new referrals, conducting initial assessments, and supporting the quality assurance of work completed by assistant practitioners and unqualified staff. The role aims to promote independence, prevent unnecessary admissions, and ensure safe and effective care planning. Hybrid working twice a week, office base 3 times a week mandatory 
£31.01ph
