Location: Slough
Working Pattern: Hybrid working
Pay Rate: £38.86 per hour

Pertemps are recruiting an experienced Occupational Therapist to join a Discharge to Access (D2A) service in Slough. This is a hybrid role offering flexibility alongside impactful frontline work.

Key Responsibilities:

• Complete timely and proportionate Occupational Therapy assessments to support safe hospital discharge
• Facilitate Discharge to Access pathways, promoting independence and recovery at home
• Recommend equipment, adaptations and short-term interventions to reduce reliance on long-term services
• Work collaboratively with health, social care and multidisciplinary teams
• Contribute to care planning that prevents delayed transfers of care and unnecessary admissions

Requirements:

• Qualified Occupational Therapist
• HCPC registration
• Experience within Discharge to Access, hospital discharge or community-based adult social care
• Strong assessment, decision-making and risk management skills
