Occupational Therapist - Connect Team Triage

Location: Enfield
Hours: 35 hours per week (Monday to Friday)
Rate: £35.38 per hour

Role Overview An experienced Occupational Therapist is required to join the Connect Team Triage Service in Enfield. The role focuses on managing urgent cases and providing timely intervention to support individuals in the community.
Key Responsibilities

• Manage urgent referrals within the Connect Team triage service
• Gather relevant information to support decision-making
• Complete functional assessments and develop support plans
• Undertake moving and handling assessments
• Carry out home visits for assessments and follow-up work
• Work collaboratively with multi-disciplinary colleagues

Working Arrangements

• Full-time post (35 hours per week)
• Community-based role with an element of hybrid working
• Monday to Friday working pattern
